Referral Instructions for Outside Providers

To submit a referral to Rheumatology Associates, please follow the steps outlined below:

1. Access the Referral Portal
   Click the following link to begin the referral process: Submit a Referral.
2. Provide Referring Provider Information
   Enter your practice and provider details. If your information is not already listed in the system, you will have the option to add it.
3. Enter Patient Demographics
   Complete all required patient information fields.
4. Upload Supporting Documentation
   Please attach the following, as applicable:
   • Most recent office visit notes
   • Pertinent laboratory or imaging results
   • A copy of the patient’s insurance card (We accept most major insurances.  We only accept Iowa Medicaid within Scott County.  We do not accept Illinois Medicaid)

Referral Review and Scheduling Process

• All referrals are reviewed by our clinical team to assess appropriateness and urgency.
• Referrals identified as urgent require direct physician-to-physician communication. Please call 563-359-4440 to speak with a rheumatologist regarding urgent referrals.
• Once a referral has been accepted, the patient will receive a secure text message confirming receipt. Our office will contact the patient to arrange an appointment after the review process is complete.
• Please note: processing and scheduling may take 2–4 weeks.
• A faxed confirmation will be sent to the referring provider once the patient’s appointment has been scheduled.
